Dermatopathology Quiz Case 178

50 year old male with a reddish blue ulcerated nodule on the left arm. The tumour cells are    CD3, CD4, and CD30 positive.  CD2, CD5, CD8 and ALK1  negative. Staining for Epstein-Barr virus is negative.

   Image 1  Low power : Cohesive, diffuse infiltrate in the dermis.  Epidermal hyperplasia is present.
 Image 2  Low power :  Section from the periphery of the main lesion.
  Image 3   High power view : Sheets of large lymphoid cells. The cells have a horseshoe or embryo shaped nuclei, prominent multiple nucleoli and abundant cytoplasm. Some reactive lymphocytes are present.
  Image 4  High power view : Atypical lymphoid cells together with some eosinophils.
 Image 5   High power view : Further section showing some atypical multinucleated cells and sheets of pleomorphic lymphoid cells.
 Image 6  High power view : The neoplastic cells and some inflammatory cells close to a blood vessel.
