Dermatopathology Quiz Case 155

Case history

A 62 year old female with a slow growing tumour on the left ankle. The tumour is 6 cm in diameter. The lobulated tumour has a variegated, tan-brown cut surface. 

Spot diagnosis:

  Image 1:  Low power view showing clusters of dilated thin-walled vessels in a pale backround. There are thrombi within the blood vessels.
 Image 2:  Higher power view showing fibrinoid change of the vessel wall and thrombus within the lumen.
 Image 3  Higher power view showing proliferation of spindle cells and sprinkling of inflammatory cells. The spindle cell at the centre of the field appears pleomorphic.
 Image 4  High power view of the stroma showing spindle shaped cells and bizarre polygonal multinucleate cells. These multinucleate cells are characterized by marked nuclear pleomorphism and hyperchromasia. Intranuclear inclusion is also present.
